Anonymous Report Form

The University’s primary concern is student safety.  As such, any individual with knowledge of sexual misconduct (including sexual harassment, rape, sexual assault, sexual exploitation, domestic violence, dating violence and stalking) involving themselves or others is encouraged to report the incident and may do so anonymously by using this form.  The University will take all reasonable steps to investigate and respond to anonymous reports of sexual misconduct and to reports in which the victim requests his or her name not be revealed to the accused.  The University’s ability to respond may be limited, including its ability to pursue disciplinary action against the accused, by the amount of information provided about the incident and the individuals involved.   

Please note that while complete confidentiality cannot be guaranteed, the University strives to maintain the privacy of all victims and informs only those who need to know.  Reporters and victims are therefore strongly encouraged to identify themselves by name and may do so on this form.   

The information you provide on sexual misconduct is used to assess campus safety, assist in providing a safe and supportive campus community, and ensure that victims are notified of all available resources and options for pursuing a complaint.

An individual may file an anonymous report by submitting this report form.  All reports will be received by both The Dean of Students Office and Office of Social Equity.  If you do not want to use this form, you may call the Title IX Coordinator at (570) 389-4529 or the Dean of Students Office at 570-389-4734 to make an anonymous report.
